2021 — incorporation
The foundation was incorporated as a Missouri nonprofit public benefit corporation in October 2021, with Dr. Gurpreet Singh Padda, MD, MBA, MHP signing as incorporator. The articles state a single purpose: the corporation is organized exclusively for charitable, educational, religious or scientific purposes within the meaning of Section 501(c)(3) of the Internal Revenue Code.

2023 — federal recognition
The Internal Revenue Service issued a determination letter dated August 29, 2023 recognizing Sevadar Foundation Inc. as exempt from federal income tax under Section 501(c)(3), with the exemption effective August 10, 2023. The letter classifies the organization as a public charity under Section 170(b)(1)(A)(vi) and states that contributions to it are deductible.
The exemption date and the letter date are different, and both matter. The letter was written on August 29, 2023; the exemption reaches back to August 10, 2023. Deductibility runs from the effective date, not the letter date.
2024 — CareGuard is listed
On November 22, 2024 the Agency for Healthcare Research and Quality listed CareGuard as a Patient Safety Organization under the Patient Safety and Quality Improvement Act of 2005. The AHRQ-assigned number is PSO P0268 and the listing runs through November 21, 2027.
That is a federal listing with real obligations attached — contract requirements, disclosure requirements, and a duty to notify AHRQ promptly if anything about the organization’s listing information changes. It is the single most consequential thing this foundation carries. What PSO P0268 means →
Since
Valor Medica and IntellaRx joined CareGuard as programs of the foundation. Valor Medica works with veterans and neighbors experiencing homelessness in the St. Louis region; IntellaRx works on opioid safety. Each has its own site and its own audience. The three programs →
As of August 2026, Sevadar Foundation has been funded entirely by its founder. It runs no fundraising campaign, employs no fundraisers and pays no one a commission on a gift.
